You may pick up the required forms at registration. However, if you want to save a little time you may print them using this page. Please follow the steps below.

These forms are for NEW PLAYERS only. If you have played with Coral Springs You Soccer in the last 3 years, you need to use a Returning Player Form from this web site.
Print the form(s). You will need a blank registration form filled out and signed for each player. Click here to load & print the form in Adobe PDF format.
Fax or mail this form and any required papaer work to us.
Be sure to include proof of residency and the players Birth Certificate or Passport.

Non Residents

Players who are not residents of the City of Coral Springs must first purchase a non-resident sports card ($172 per family) from the Parks & Recreation Department. This is located in the Gymnasium at 2501 Coral Springs Drive.
